Goalkeeper/Striker Camp

Dedicated on October 17, 2002, the Tinsley Center is a 84,000 square foot facility.  The facility is home to the Department of Athletics and Recreation, the Department of Movement Arts, Health Promotion and Leisure Studies, the Thornburg Fitness Center, as well as faculty/staff and coaches offices. The Center also houses five academic classrooms and three laboratories; Exercise Science, Biomechanics and Athletic Training. Eight locker rooms accomodate varsity sports teams, faculty/staff, and students. The Equipment Issue, Sports Medicine Center and Conference Room are within the Tinsley Center.

The Swenson Complex is equipped with a Olympic 9 lane track, Football field w/ seating 1500.  Also, SwensonPitch has Three field designated for Field Hockey / Women’s Lacrosse, Soccer game and practice field.

I CERTIFY THAT MY CHILD IS IN EXCELLENT PHYSICAL HEALTH AND MAY PARTICIPATE IN STRENUOUS PHYSICAL ACTIVITIES, INCLUDING
SOCCER TO BE PLAYED AT CAMP. PERMISSION IS GRANTED FOR MY CHILD TO RECEIVE EMERGENCY MEDICAL TREATMENT IF NEEDED.
I HEREBY RELEASE MPS AND ALL THEIR AFFILIATED ENTITIES FROM AND AND ALL LIABILITY CLAIMS, DEMANDS AND CAUSES OR ACTION
TO PHYSICAL INJURY, PROPERTY DAMAGE AND/OR OTHER LOSS SUFFERED BY MY CHILD IN CONNECTION WITH HIS/HER PARTICIPATION
IN THE CAMP. I AGREE THAT YOU MAY PHOTOGRAPH MY CHILD DURING CAMP AND THAT YOU RETAIN THE RIGHTS TO USE THESE VISUAL
IMAGES IN ANY MANNER YOU WISH WITHOUT COMPENSATION TO MY CHILD. I CONFIRM THAT I AM A PARENT/ GUARDIAN OF THE
MINOR NAMED ABOVE AND I AND THE MINOR ABOVE AGREE THAT THE GRANT AND RELEASE CONTAINED THEREIN BINDS ME AND THE
MINOR TO ALL OF ITS TERMS.

APPLICATION - MANDATORY • ALL OF OUR CAMPS COMPLY WITH REGULATIONS OF THE MASSACHUSETTS DEPARTMENT OF PUBLIC HEALTH AND ARE LICENSED BY THE LOCAL BOARD OF HEALTH. EVERY CAMPER MUST HAVE A PHYSICAL PRIOR TO PARTICIATION IN ANY MPS CAMP. A COPY OF THE PHYSICAL WITH IMMUNIZATION RECORD MUST BE SENT IN WITH THE APPLICATION FORM.

YOU MUST PROVIDE AN IMMUNIZATION RECORD TO PARTICIPATE IN THE CAMP AS PER BOARD OR HEALTH REGULATIONS.
