Physical & Immunization Records - A
copy of your child’s up to date physical / immunization records, and
a signed waiver form must brought to the first day of camp in order to be allowed to participate.
FAILURE TO PROVIDE THESE WILL RESULT IN YOUR CHILD NOT BEING ABLE TO
PARTICIPATE IN THE SESSIONS
